For managing complex guardianship cases, we have found nursing expertise to be particularly valuable. Our clients’ functional, cognitive, legal, and family issues are deeply intertwined. Our Director of Nursing, Edwin Nunez, RN, provides practical day-to-day clinical perspectives that help guardians, attorneys, courts, and families make informed care decisions.
Nursing expertise is especially effective when integrated with medicine, social work, and case management—as our team does. Medical providers diagnose and direct treatment while nurses monitor how these plans are actually working in the home. Social work addresses family dynamics, resources, and psychosocial concerns. Case management coordinates services and systems. This interdisciplinary approach enables us to provide organized and responsive care.

Nurses are essential to high-quality home-based care, yet are underutilized in community-based care. Individuals receiving care at home often have overlapping needs that may involve chronic illness, polypharmacy, mobility or cognition challenges, nutrition, skin integrity, and caregiver support.
As the long-time Director of Clinical Services at Wellspring Personal Care, Ed Nunez is uniquely trained to assess these interconnections, recognize subtle changes, and respond before concern becomes crisis.

The Complex Guardianship Support team provides our support on a fee-for-service basis, which allows us to offer the dedicated time, expertise, and continuity that complex situations deserve. This model helps us remain independent, responsive, and focused entirely on the needs of the individuals and families we serve.

Our team is committed to treating every person with dignity and respect, and we gladly assist in identifying community resources or funding options for those who may need additional support.
