0-1: Pre-frail. The condition for the positive score should be evaluated for treatment, prevention and further deterioration.

2-3: Frail. Slowed and evident impairment in daily living and self-care. At risk for further impairment and disability.

3-4: Moderate frailty. Needs assistance with maintaining home, nutrition and medications. At risk for falls, dehydration, acute illness and injury.

5: Severely frail. Requires daily care and supervision. Highest risk for injury, hospitalization and death.
