Home Nursing Care
Wellspring Personal Care provides in-home skilled nursing care to fit your needs when and where you need it, including the transition from hospital or rehab facility to home. Our ability to care for the most challenging populations with complex medical and cognitive issues has made us the provider of choice for high-risk clients.
Our director of clinical services will conduct a comprehensive in-home nursing assessment and help you customize services to meet your specific needs. We ensure you have input in developing a plan of care for yourself, your family member or your client.
Adult Care
Wellspring’s skilled nursing team includes registered nurses (RNs), licensed practical nurses (LPNs) and certified nurse assistants (CNAs). The patient’s Personal Care Team receives ongoing support from our medical director, a licensed physician, and Wellspring’s founder, a licensed clinical social worker.
We offer an array of skilled nursing services that include:
- Enteral feeding
- Ostomy care
- Tracheostomy and ventilator care
- Medication administration and monitoring
- Reporting changes in condition
- Immobility care
- Use of adaptive equipment
- Personal care
- Meal preparation and feeding
- Toileting
- Incontinence care
We have expertise in caring for patients with a wide range of conditions, including:
- Alzheimer’s disease (including early onset)
- Lewy body dementia (LBD)
- Frontotemporal dementia (FTD)
- Brain injury
- Parkinson’s disease
- Congestive heart failure
- Diabetes management
- Mental illness
The Wellspring team provides services in the home, in an assisted living or nursing home facility, or bedside during a hospital stay.
Pediatric Care
At Wellspring Personal Care we have extensive experience caring for fragile children with multiple chronic conditions, and have helped maintain our young clients at home through adulthood.
Our pediatric services include but are not limited to:
- Gastrostomy care

- Monitoring and recording vital signs

Wellspring Personal Care’s clinical management team works collaboratively with case managers and other health care providers, while engaging parents in problem solving and maintaining compliance with the plan of care. Our pediatric nursing field staff includes licensed practical nurses (LPNs) and certified nurse assistants (CNAs).
Home Care MetricsSM
Home Care MetricsSM is our proprietary clinical monitoring system. For adult and pediatric clients with health conditions that require monitoring, Wellspring Personal Care equips the home with easy-to-use tablet computers. Caregivers document services delivered and monitor and track metrics related to the client’s unique healthcare condition.
Billing
All Wellspring Personal Care services are private pay. RN/LPN skilled nursing is offered by the hour. CNA services are available by the hour or live-in.
Wellspring Personal Care is licensed by the Illinois Department of Public Health for both home nursing and home services.
