What happens when a hospital is ready to discharge a patient who still needs ongoing clinical care? Typically they’re transferred to a nursing home or nursing and rehabilitation center. Most people would rather be sent home (or wherever they call home) so they can continue living in their own space. Fortunately, clients who choose Wellspring can do just that.
Wellspring has always been ahead of the curve when it comes to keeping people safe at home. We are known for our expertise in high-acuity care, meaning that even clients who need an intensive level of care can continue living at home safely. Our medical director and director of clinical services ensure that our in-home caregivers are thoroughly trained in providing clinical care such as enteral feeding, ostomy care, tracheostomy and ventilator care, medication administration and monitoring, and more. All care is managed by our director of clinical services, who is a registered nurse, in consultation with the client’s healthcare providers.
Our expertise in clinical care is what sets us apart from other in-home care agencies. Most of the others, including the chains, do not offer nurse-supervised care and are not equipped (or licensed) to provide clinical care in the home. They typically provide companionship, chores, meal preparation and transportation to appointments. They also provide medication reminders, but they are not legally permitted to administer medications or to provide the more specialized care that Wellspring can.
You may have seen news about the “hospital at home” trend, as hospitals grapple with staffing challenges, the aging population, and the growing number of people who have acute health problems. Some hospitals are creating programs where patients are discharged to their home, rather than a nursing home, and receive nurse-directed care, based on their doctor’s orders.
This is a great service for people who want to recover at home or simply continue living at home rather than going into a nursing home or assisted living.
