Wellspring Personal Care provides nurse-supervised home care to ensure safety, health, and daily support. Consultants on Disability offers expert case management to coordinate services, monitor outcomes, and meet legal and financial requirements. Together we streamline communication among care providers, guardians, and family members, delivering a coordinated, efficient, and accountable approach.

Case Example 1: Complex Medical Needs
Challenge: A court-appointed guardian was over-whelmed managing a client with multiple chronic illnesses, frequent medication changes, and repeated hospitalizations.
Our solution: Our nurse and physician team reviewed the medical history, streamlined medications, and worked directly with the client’s specialists. The case manager coordinated follow-up appointments and ensured home health services were in place.
Outcome: Hospital visits were reduced, the client’s health stabilized, and the guardian had clear, reliable updates to inform legal and financial decision-making.
Case Example 2: Social Isolation & Safety Risks
Challenge: A trustee managed the estate of an elderly client living alone. The client was isolated, declining cognitively, and repeatedly calling emergency services for minor concerns.
Our solution: Our social worker assessed the home environment, arranged community support, and helped transition the client into an assisted living facility. The case manager maintained ongoing oversight to ensure continuity of care.
Outcome: The client became more socially engaged and safer, while unnecessary emergency service use was eliminated.

Challenge: A legal guardian was caught in the middle of family disagreements about the care of an adult client with disabilities.
