Wellspring Personal Care and Consultants on Disability are excited to announce our new collaboration, Complex Guardianship Support. This new collaboration brings our complementary strengths and over 120 years of experience together to provide a full spectrum of services for individuals with complex guardianship needs.
Wellspring Personal Care provides nurse-supervised home care to ensure safety, health, and daily support. Consultants on Disability offers expert case management to coordinate services, monitor outcomes, and meet legal and financial requirements. Together we streamline communication among care providers, guardians, and family members, delivering a coordinated, efficient, and accountable approach.
Our affiliation combines our collective years of experience. Our skills bring together disability expertise, case management, social work, nursing, and medicine. Our work together over 30 years has provided crucial support for both the most vulnerable individuals and the legal community, and we have a unique understanding of the probate system.
We provide:
- Integrated assessment, planning, and coordination for individuals with multifaceted medical, behavioral, or social needs.
- Streamlined communication among care providers, lawyers, and family stakeholders.

Our services are highly individualized. They can be long-term or short-term. Each client assessment is reviewed by all of the team, and solutions are based on our collective knowledge. We develop care plans that prioritize the client’s needs and wants. Services are identified from our large database of ancillary providers, all of whom we have vetted. In addition, we are available to coordinate and monitor care.
