Wellspring Personal Care strives for excellence in patient care and employs a professional management team with the training and expertise to guarantee quality service. Wellspring is bonded and insured and complies with all state and federal labor laws. Our caregivers are licensed nurses, certified nurse assistants, or experienced home health aides. They are carefully selected for assignments based on skill level and client needs.

The following is a list of services provided:

  • Personal Care

    Assistance with all activities of daily living, companionship, dementia care, medication reminders, shopping, light housekeeping, and meal preparation. Services may be billed hourly or at a live-in rate (depending on client needs).

  • End of Life Care

    While we do not replace the hospice services available through Medicare and Medicaid, our private duty services include hospice trained LPNs, CNAs and Home Health Aides to supplement this specialized care.

  • Nursing Home, Assisted Living and Hospital sitting

    When illness or an accident causes hospitalization, having someone there may make the difference between a long stay and an earlier discharge. Wellspring staff can provide care in nursing homes, assisted living facilities, and hospitals as well as in private homes.

  • Case Management and Service Coordination

    Assisting older people, disabled people and their families to attain the highest quality of life given their circumstances.

    • Conduct care planning assessments to identify problems, and provide solutions.

    • Support patients and their families as they navigate the healthcare system; decrease the fragmentation of services; encourage health team communication; and contribute significantly to patient safety, quality of life, and cost-efficient management of resources.

    • Review financial, legal, or medical issues and develop care plans that ensure client safety while conserving financial resources.

    • Provide crisis intervention.

    • Act as a liaison to families at a distance, overseeing care and quickly alerting families to problems.

    • Assist with moving an older person to or from a retirement complex, assisted care home, or nursing home.

    • Provide consumer education and advocacy.

  • Recovery from Elderly Abuse or Exploitation

    Since 1993, Wellspring Personal Care has been called in to provide in-home care for victims of elder abuse or financial exploitation. Our “Rapid Response Team”, comprised of our RN Clinical Director and carefully selected caregivers, does whatever is necessary when we assume responsibility in these recovery cases.

    To ensure client safety and to prevent future occurrences of exploitation or undue influence, Wellspring provides a complete support system to anyone involved in the case: family, attorneys, trust officers and law enforcement. We work closely with bank trust departments and provide detailed financial reporting and supervision of expenditures.

    Through aggressive case management (long term strategic and immediate planning related to the specific issues), even our most fragile victims of financial abuse have been able to remain safely in their own homes.
